Which SimpleMap example? The basic SimpleMap doesn't use a MapHandler.

You should be adding a LayerHandler to the MapHandler, and adding
layers to the MapHandler or LayerHandler.

I think it's related to how your application is configured. I can't
replicate your problem, on Windows or Mac.
